POSITION TITLE:
Transition Specialist
JOB DESCRIPTION:
Primary Purpose: Responsible for facilitating the planning process and implementation of individual transition plans for handicapped students in secondary schools.
RESPONSIBILITIES:
Major Responsibilities and Duties
- Be proficient with federal and state transition rules and regulations, acting as the district contact for transition.
- Conduct student and parent interviews regarding post-secondary goals.
- Provide staff development to staff, students, and parents concerning issues related to transition services.
- Attend ARD meetings as necessary.
- Establish and maintain contact with various community agencies and post-secondary institutions in order to assist students in transitioning from public school to post-graduation environments.
- Attend community meetings regarding the transition process (state and federal updates).
- Maintain district student files on transition and assist in preparing required documentation for monitoring.
- Collaborate with middle school, junior high, and high school counselors on students' four year plans and their career pathways.
- Collaborate with the Career and Technology Education (CTE) Department regarding course sequences and vocational experiences.
- Responsible for preparation and submission of State Performance Plan (SPP Indicator 13) related to transition development for secondary students served within special services.
- Collaborate with campus principals, program leaders, and supervisors as needed.
- Must be proficient in the Districts' computerized individualized educational Plan (IEP) transition supplement and be able to train district staff.
- Establish job sites for all high schools and the Adult Transition Program staff.
- Review secondary student files to determine which agencies should be involved in the individual transition planning (ITP) process for each student in grades 7-12.
- Assist with follow-up studies on students who graduate.
- Other duties as assigned by the supervisor or designee.

EXPERIENCE:
Minimum Experience:
- 3 years of teaching experience
QUALIFICATIONS:
Minimum Education/Certification:
- Bachelor's degree in content and/or related field from an accredited college or university
- Valid Texas teacher certificate or teaching certificate from out of state appropriate for position
- Valid Texas driver's license
- Eligible for coverage under the District's vehicle insurance program
Special Knowledge/Skills:
- Knowledge of federal and state special education law
- Understanding of the individual needs of special needs students
- Ability to communicate with all levels of special needs students and their parents
- Ability to interpret policy, procedures, and data
- Ability to coordinate campus functions
- Strong organizational, communication, and interpersonal skills
- Constructive and cooperative attitude toward the duties of the aide, the school, the staff and the students
