Responsibilities
- Provide comprehensive primary care services to patients of all ages, including preventive care, acute care, and chronic disease management.
- Conduct thorough patient assessments, develop individualized treatment plans, and provide patient education.
- Order and interpret diagnostic tests, prescribe medications, and perform minor procedures.
- Collaborate effectively with other members of the healthcare team, including physicians, nurses, medical assistants, and behavioral health providers.
- Maintain accurate and timely patient records in accordance with HIPAA and other regulatory requirements.
- Participate in quality improvement initiatives and contribute to a culture of continuous learning.
Required
- MD/DO, NP, or PA degree from an accredited program.
- Current and unrestricted license to practice in [State].
- Board certification in Family Medicine (MD/DO) or national certification as an NP or PA.
- Current BLS/ACLS certification.
- Excellent communication and interpersonal skills.
- Strong clinical and problem-solving skills.
- Commitment to serving underserved populations.
- Ability to work effectively in a team-based environment.
- Experience working in an FQHC or community health center setting.
- Experience with electronic health records (EHR) systems, specially eClinicalWorks.
