What are the responsibilities and job description for the Social Worker position at Lancaster Nursing and Rehabilitation Center?
The Social Services position provides medically related social services to our residents, to attain and maintain the highest practical physical, mental and psychosocial well- being of each resident.
Under the supervision of the Social Service Director, maintains the Social Services Department based on the needs of the type of resident's care in the faculty, including the community agencies to provide economic, psychological, and medical assistance.
Assists in planning, developing, organizing, implementing, evaluating and directing social services programs of the facility in accordance with the current federal, state, and local standards, guidelines and regulations.
Develops a social history and assessment care plan on each resident. Involved with the admission and discharge process.
Assist in development, administration, and coordination of department policies and procedures in conjunction with facility standards and committees.
Ability to listen and act compassionately with residents and family members to address concerns appropriately and within a timely manner
Must possess at a minimum a bachelor's degree from accredited school in social work or related field.
Long Term Care experience preferred.
Excellent communication and computer skills a must
