Overview:
The primary purpose of the job position is to assist in planning, organizing, developing, and directing the overall operation of Social Services Department in accordance with current federal, state, and local standards, guidelines, and regulations, our establish policies and procedures, and as may be directed by Social Services Director and/or Administrator, to ensure that the medically related emotional and social needs of residents are met/maintained on an individual basis. As Social Worker, you are delegated the authority, responsibility, and accountability necessary for carrying out your assigned duties.
Duties & Responsibilities:
Assist in planning, developing, organizing, implementing, evaluating, and directing social services programs of facility
Participate in community planning related to interests of facility and services and needs of resident and family
Perform administrative requirements, such as completing necessary forms, reports, etc. and submitting such as required
Be prepared to handle emergencies as they arise (i.e. rescheduling work assignments and work schedules, etc)
Meets with Social Services personnel, on a regular basis, and identify and correct problem areas, and/or improve services
Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the unit and shift.
Participate in resident care plan conference and is active member of care plan team
Education & Qualifications:
Must possess at a minimum a bachelor’s degree from accredited school in Social Work or related field.
A physical examination and/or evidence from a physician indicating freedom of communicable disease to be available upon employment.
Must be in good physical and mental health.
Must be knowledgeable of social services practices, procedures, laws, regulations and guidelines governing long-term care
Ability to work independently or part of a group.
Effectively communicate with others.
Experience:
Previous long-term care experience is desirable. Past supervisory experience preferred. Computer experience preferred.
