Landmark Care Center and Rehab is Yakimas Valley's premiere long-term care and rehabilitation facility. Our residents build strength and improve mobility in a fully equipped rehabilitation gym with therapists from all disciplines. Long-term care residents have access to a restorative program to maintain their abilities.
Conveniently located near the highway in a modern and well-cared for facility, we are located at 710 N. 39th Avenue in Yakima.

Job Duties include:
- Provides physical therapy services to residents and initiates referrals when appropriate and in accordance with Physical Therapy Department Procedures.
- Conduct screenings of residents at regular intervals to determine the need for intervention/treatment.
- Assess residents to obtain data necessary for treatment planning and implementation and conduct specialized evaluations as indicated.
- Interprets and communicates evaluation findings to residents, families and team members.
- Establishes individual resident schedules in conjunction with other team members
- Follow treatment plans developed by evaluating therapists identifying a target list, including short term and long-term goals and methods to achieve identified goals.
- Coordinates treatment plan with resident, family and other team members.
- Implements and conducts treatment as outlined in treatment plan.
- Monitors residents response to intervention and modifies treatment as indicated to attain goals in accordance with physician orders.
- Develop appropriate home or community program to maintain and enhance the performance of the residents in their own environment.
- Documents results of residents assessments, treatment, follow-up and termination of services.
- Adheres to department and resident schedule and modifies as appropriate for the treatment regimen.
Must have graduated from a Physical Therapist Assistant Program
Must have a Physical Therapist Assistant license in the State of Washington.
