Landmark Care Center is looking for a compassionate person to assist our Social Services Department
In this role you will coordinate services offered by the Social Services Department and promote the psychosocial growth and well-being of residents, family and staff. Assist residents and their families in finding and utilizing community resources such as financial, legal, mental health, and transportation. Work within an interdisciplinary team that provides nursing care, rehab and well-being for residents that are staying with us while they recover or have chosen Landmark as their forever home.
In this position you will engage with residents and build rapport to complete an array of assessments that help us to learn about their mental and social health. You will collaborate with our interdisciplinary team to provide the best, comprehensive care possible. You will also serve as a liaison for resident's family to further an open and reciprocal flow of information.

This job will be approximately 20 hours per week. Days and times will be between Monday and Friday during regular business hours with some flexibly which can be discussed during the interview.

Education/Training: High school diploma or equivalent required. BA in Social Work or related field would be helpful.
Skills: Ability to deal tactfully with co-workers, residents, family members, visitors, and the public. Must have the ability to work with ill, disabled, elderly, emotionally upset, and, at times, hostile people within the facility while remaining calm and professional. Must have strong communication skills and an ability to listen to and empathize with others.
Experience: Long term care experience or health care social work preferred.
