Summary
We are seeking a Scheduling Coordinator to join our team at Landmark Home Health. This role is crucial for ensuring the efficient scheduling of home health services for our clients.
Responsibilities
- Schedule In-Home Service workers to meet participant’s Care Plans.
- Prepares the daily, weekend and holiday schedule.
- Completes and monitors all In-Home Service staff and participant schedules. Make revisions as necessary.
- Accepts referrals for services, enters as prospective client in EMR and enters name on assessment log to be scheduled.
- Delivers all required communications with participants Service Coordinators and Sam’s program.
- Uploads all communications in participant charts in EMR.
- Responsible for making participant admission packets with required documentation for each program.
- Maintains and protects the confidentiality of all information, participants and proprietary.
- Reports to the Manager with any scheduling discrepancies.
- Communicates pertinent information daily to the Manager and Service Coordinator for each participant.
- Communicates with Manager of In-Home Services regarding the changing staff needs for services.
- Responsible for increasing number of participants with each program.
- Writes up Transport referrals, log them, ensure credit cards are charged day before transport and assigns staff to transport.
- Perform transport(s) if needed.
- Completes monthly chart audits.
- Complete biweekly Payroll for Direct Care Workers.
- Acts as a backup for billing.
- Participates and helps to prepare for State and County audits.
- Supports the Manager of In-Home Services in following up on projects and deadlines.
- Perform other duties as assigned by the Manager of In-Home Services and/or the Administrator.
Requirements
- High School diploma
- Valid current driver’s license and necessary insurance as mandated by the state of Pennsylvania.

Job Type: Full-time
Pay: $19.00 - $19.50 per hour
Expected hours: 40 per week
