JOB TITLE: Personal Care Specialist
DEPARTMENT: Nursing
Category: Hourly/Nonexempt
REPORTS TO: Wellness Director
GENERAL PURPOSE: To be part of a vibrant team devoted to delivering exceptional care that promotes dignity and choice for all of our residents.
RESPONSIBILITIES:
- This position is a Safety Sensitive Position. The essential functions, physical demands and mental competencies of this job require the employee to maintain the ability to work in a constant state of alertness safely.
- Assist residents with performance of activities as indicated in service plans.
- Assist residents with performance of simple treatments as indicated in service plans.
- Assist residents with emotional health and/or problem behavioral issues as indicated in service plans.
- Perform other services for residents as indicated in service plans (e.g., laundry and pet care). Note: any team member with potential risks related to pet care (e.g., allergies and pregnancy) should exercise caution in this regard and contact his or her supervisor concerning any related issues.
- Assist in development and revision of service plans by informing supervisor of current resident needs and preferences, as well as successful service approaches.
- Observe and report resident problems (as listed in the Observation and Reporting sections of Personal Care Specialist Resource, or in-service plans) and report such problems to LPN (if on duty) or Medication Tech/Aid.
- Attend and participate in onboarding, annual and in-service training and team member meetings, as requested.
- Adhere to all company policies and procedures.
- Perform other duties as assigned by the Wellness Director.

SKILLS and QUALIFICATIONS:
- Must be at least 18 years old.
- Demonstrate ability to communicate effectively in English, both verbally and in writing.
- Meet state or provincial health related requirements.
- Maintain any certification as required by state or provincial regulations.
PHYSICAL REQUIREMENTS:
Physical Demands include: Standing, Walking, Sitting, Bending, Reaching, Pushing, Pulling, Climbing Stairs, Grasping, Manual Dexterity, Stooping, Kneeling, Crouching, Vision Acuity, Talking, and Hearing. The weight determined to be appropriate for a Personal Care Specialist to lift with or without reasonable accommodations is up to 50 lbs. EXPOSURE RISK CLASSIFICATION: May perform unplanned tasks involving exposure to blood, bodily fluids, or tissue which may contain AIDS, HIV, and Hepatitis A, B, and C viruses.
