Responsibilities
The Intake Coordinator monitors & provides accountability to patients in treatment and embarking on their road to recovery. They are the main staff point of contact for all patients entering the facility. They monitor patients regularly, resolving conflicts and responding to critical situations. They interact regularly with other clinical staff to aid in the recovery process. Our Intake Coordinator:
- Stationed in Lobby; at least one must be present at all times during assigned hours
- Only team member permitted to conduct new patient intakes
- Monitors Intake Thread for Incoming Patients
- Greets patients outside lobby door; for patients arriving by Uber, sets alarm and interrupts other intakes to greet patient
- Confirms patient arrival within 5 minutes on Intake Thread
- Immediately gathers patient phone, valuables for safe storage during intake
- Tags patient luggage; ensures lead PES puts luggage in luggage room within 10 minutes of patient arrival
- Completes consents and intake paperwork
- Reviews Welcome Packet, provides towels and other critical items to patient
- If two are present, and facility has fewer than 5 intakes scheduled, one may be reassigned if other PES call off shift
- Completes UDS and Skin Search on patient
- Perform all other tasks as assigned by supervisor
- Work as a highly dependable, independent member of the treatment team
- Work cooperatively with all levels of employees and management
- Are committed team players willing to learn new skills
- Reports directly to the Patient Engagement Manager
