Discharge Planner / Social Services Coordinator Needed at Langdon Place of Keene
Langdon Place of Keene is looking for a compassionate and organized Discharge Planner / Social Services Coordinator to join our team! As we begin a new chapter under new ownership, we are dedicated to providing exceptional care to our residents, and we need a skilled individual to help support our residents through the discharge planning process.
Key Responsibilities :
- Develop and implement comprehensive discharge plans for residents, ensuring a smooth transition from the facility to home or other care settings
- Assess residents’ social, emotional, and financial needs, and coordinate appropriate services and resources
- Collaborate with interdisciplinary care teams, physicians, and family members to facilitate discharge planning
- Advocate for residents and their families, providing guidance and support during the discharge process
- Ensure proper documentation and compliance with state and federal regulations
- Provide crisis intervention, counseling, and emotional support to residents and families as needed
