We are seeking an experienced Case Manager for Lantern's Supportive Housing in New York, NY. The Case Manager will be expected to assist this population in stabilizing their lives, maintaining their housing stability and attaining their physical, emotional, vocational, and social goals. Case Managers are responsible for assessing the needs of the client, linking clients to services, planning a course of action towards stabilization and re-housing. They will also be monitoring progress, and documenting client contacts.

Essential Functions
- Provide services ranging from concrete services (benefits and entitlements) to case assistance and management, group programming, individual counseling/support, advocacy, resources and referrals, for tenants.
- Utilize a harm reduction approach to support a diverse caseload of formerly homeless individuals around issues of mental health and substance use.
- Actively engage residents through regular apartment visits, community outings and escorts to appointments as needed.
- Develop safety plans for residents and work with the team to deescalate clients and collaboratively manage crisis as they arise.
- Maintain current documentation required for all cases, from all agencies necessary for reporting purposes, including progress notes, case contacts and all case summaries required.
- Participate in staff and clinical meetings and individual supervision to ensure best practices in the performance of role responsibilities.
- Bachelor's degree required.
- One year of experience working with single adults impacted by poverty, chronic homelessness, mental illness, substance abuse and/or medical issues required.
- Highly organized and able to manage a diverse case load of clients with diverse needs.
- Experience with electronic case management systems.
