What are the responsibilities and job description for the Adult Mental Health Case Manager position at Lapeer County Community Mental Health?
Significant Duties:
Manage a caseload of adults in collaboration with their therapists. Work toward treatment plan compliance and help support the consumers as they interact with other community agencies and institutions. Knowledge of Michigan’s Medicaid programs.
Qualifications:
LBSW preferred, but will consider experienced or highly motivated, self-starting individuals with Bachelor’s Degree in social work, psychology or related field. Knowledge of local resources and social work license preferred. Experience working with Adults Preferred. Valid Michigan driver’s license required.
Entry Level, then 6 month increase, step 1, step 2, step 3 and 4 as follows:
Salary: Entry: $ 49,857.60 6 months: $53,601.60 1st Year: 57,345.57 2nd Year: $61,845.02 3rd Year: $66.321.98 4th Year: $71,136.40
