ABOUT THE POSITION:

The Case Manager is hired and supervised by the Director of Substitute Care. They are responsible for coordinating and facilitating ongoing supports and services to children and youth experiencing severe emotional/behavioral disturbance and their families. They will engage in extensive collaboration with other agencies and supports serving children and youth in need. They share responsibility for supervision of support staff.

Essential Duties & Responsibilities:

All direct-services positions necessitate training and preparedness in physical restraint techniques, given the at-risk population served by the employer.

  • Provide ongoing support and services to children and youth experiencing severe emotional/behavioral disturbance and their families. Provide support and supervision to therapeutic foster care providers. Provide crisis intervention as needed.
  • Participate and facilitate ongoing treatment team meetings, SRS Case Plan Reviews, educational meetings, etc.
  • Develop and implement treatment plans which provide for the delivery of services and interventions as identified by the treatment team.
  • Assist in developing natural supports for children and youth in within their communities
  • Work collaboratively with Department of Children and Families (DCF), schools and other agencies.
  • Maintain client records to include writing monthly summaries, treatment team summaries and all other required paperwork relevant to providing client services.
  • Attend all staff meetings
  • Prepare agenda for team meetings
  • Train and supervise staff
  • Frequent travel to foster homes and residential homes
  • Other duties as assigned

Financial, Supervisory & Financial Responsibilities:

  • Review all receipts for client clothing, recreation and food to ensure spending is within budgeted amounts; submit timely.
  • Ensure all prescriptions are filled
  • Update medication records and provide notifications of change
  • Prepare medication for home visits and for school dispensing
  • Follow protocol for disposal of all outdate medications
  • Attend staff meetings, scheduled supervision, and trainings as assigned
  • Prepare agenda for team meetings
  • Train and supervise assigned staff

Physical Requirements:

  • Given the at-risk population served, the position requires ongoing training and preparedness in the execution of physical restraint techniques. This may require running to catch a child. The “Handle with Care” training program used articulates that individuals with back and neck limitations are likely to be hampered in the execution of the protocols and greatly increases risk of harm to self or student.
  • Potentially long hours of sitting and driving in various types of weather and different times of the day to transport clients.

Qualifications:

  • Required: BA in Human Services or related field.
  • Required: Valid driver’s license, excellent driving record
  • Required: Access to safe, reliable, insured, inspected vehicle for transportation of clients.
  • Preferred: Two years of human service delivery with children and families preferred.
