We are seeking a dedicated and experienced long-term care MDS nurse to join our team. As an MDS, you will play a crucial role in ensuring accurate and timely completion of Minimum Data Set (MDS) assessments for our patients. Responsibilities :
- Conduct comprehensive assessments of patients' health status and develop individualized care plans
- Collaborate with interdisciplinary team members to ensure accurate and complete documentation
- Monitor and update MDS assessments as required by regulations and changes in patients' conditions
- Ensure compliance with all state and federal regulations related to MDS assessments
- Coordinate with healthcare providers to obtain necessary medical documentation for accurate coding
- Participate in quality improvement initiatives to enhance the overall care delivery process
- Provide education and training to staff regarding MDS assessment processes and requirements
