ESSENTIAL JOB RESULTS
1. PROVIDES INFORMATION, TRAINING AND SUPPORT TO PATIENTS CLIENTS, AND FAMILIES by performing outreach to the community, receiving client referrals, conducting or assisting in educational efforts and responding to requests for support and guidance.
2. OBTAINS REQUIRED INFORMATION by interviewing applicants, family members and others; completing applications; confirming information for accuracy; and gathering documents such as forms, regulations and guidelines, and other materials.
3. INCREASE PROGRAM VISIBILITY by performing outreach in the community, participating in community education initiatives, and providing collaboration and referrals with community providers.
4. ASSISTS INDIVIDUALS AND FAMILIES IN ACHIEVING AND MAINTAINING WELLNESS by being available to program clients, with such activities as nutrition counseling, support and training in the use of medication or treatment supplies.
5. EDUCATES APPLICANTS / CLIENTS by explaining physiological processes, diet and nutrition options, and resources for service and support, including financial support.
6. OBTAINS ASSISTANCE FOR APPLICANTS / CLIENTS by referring them to community programs and resources.
7. COMPLIES WITH FEDERAL, STATE, AND LOCAL LEGAL REQUIREMENTS by studying existing and new legislation; enforcing adherence to requirements; informing management of needed actions.
8. PROVIDES BACKUP SUPPORT TO THE DIRECTOR OF NURSING by actively participating in leadership activities with the Director of Nursing and by assisting in developing, monitoring, and complying with the budget for the clinical operations and participating in quality improvement initiatives.
9. MAINTAINS CLIENT CONFIDENCE AND PROTECTS OPERATIONS by keeping information confidential.
10. CONTRIBUTES TO TEAM EFFORT by accomplishing related results as needed.
JOB QUALIFICATIONS AND JOB EVALUATION DOCUMENTATION
EDUCATION
KNOWLEDGE : Must have a high school diploma or equivalent.
Experience : Experience in Public Health or Social Work is recommended; however, not a requirement.
Must have a valid driver's license, auto insurance and a dependable vehicle.
Skills and Abilities :
- Ability to communicate effectively in speaking, reading and writing of both English and Spanish.
- Oral and written communication skills, including effective communication in person and on the telephone.
- Ability to exercise poise and tact in all contacts inside and outside LCDN.
- Supervisory skills and knowledge of diabetes management techniques an some familiarity with state and federal laws and regulations for the operation of the diabetes program.
- Ability to interact with people in a culturally sensitive manner.
- Ability to speak effectively in public forums and small groups and facilitate discussion in small groups.
- High performance standards are expected by fulfillment of some continuing education and current certification in CPR with reasonable time frame after employment..
- Must be able to function in remote areas and be sensitive to cross-cultural medical situations.
