Description
The Day Program Service Coordinator (DPSC) is responsible for providing person centered case management services for all assigned day program participants. This includes coordinating services with referral source personnel, accessing community supports, developing Individual Service Plans for each individual, ensuring programs and staff provide quality services adhering to the Individual Program Plan and the Individual Service plan, monitoring the progress of individuals served and making adjustments in the plan as necessary, and all other case management related services. Additionally, the DPSC is responsible for ensuring person centered planning and thinking principles and tools are used to develop Individual Service Plans as appropriate for each person served.
The DPSC will assist staff in developing and implementing activities and curriculum that ensure participant goals are being addressed. This includes updating appropriate staff on updates to participant’s plans, helping create training schedules related to their goals, and ensuring goal data is documented. The DPSC will also develop and foster relationships with community members and organizations that help give opportunities for participants to accomplish their goals. They will also explore new curriculum, technology, and innovative approaches that will help improve the services that program provides. The DPSC will coordinate with direct service staff, other day program leadership staff, consultants, and participants to organize participants’ daily schedules and routines.
The DPSC along with all day program leadership staff are responsible for providing a safe, healthy and supportive environment for the people served. They shall ensure that participants are afforded their rights and are treated with dignity and respect at all times, especially during their most difficult and/or vulnerable moments. The DPSC is responsible for monitoring and addressing as appropriate any medical and/or physical needs of the people served.
The DPSC will support the day program leadership team on an as needed basis with staff management.
Essential Duties
- Ensure the care, welfare, and safety of the people served by the program.
- Act as an interdisciplinary team member to ensure services meet the needs of those served by the program. This includes working directly with appropriate co-workers, case managers, residential providers, family members, other members of the individual’s circle of support, and most importantly the program participant.
- Ensure safety protocols are being followed regarding PPE use, infection control, cleanliness of facilities, and all other safety related regulatory requirements.
- Ensure clear lines of communication and cooperation between other departments.
- Have clear knowledge of California Code of Regulations Title 17 and Title 22 and ensure that day programs are in compliance with regulations.
- Uphold the policies and procedures of the agency including, but not limited to, budget, fiscal responsibility, and safeguarding the resources and property of the agency and the people served by the agency.
- Adhere to principles of the Las Trampas mission statement.
- Serve as an advocate for people served by the agency and increase opportunities for the improvement of their lives.
- Foster a culture that encourages and reinforces person centered practices.
- Maintain participant confidentiality including not disclosing personal information about the people we serve to community members or others who do not have a direct need to know such information.
- Contribute to teamwork and a positive workplace environment.
- Ensure the rights and dignity of the people served by the agency in all situations.
- Actively recruit and evaluate new participants for potential enrollment in the day program. Work with the participant, their families/caregivers, and the referral source to on-board any appropriate candidates.
- Provide person centered case management services to the people served by Las Trampas day program.
- Maintain all records for people served by the program. This includes ensuring all file materials are up to date, online data management systems reflect current information, and staff members have access to the most current information regarding each participant.
- Ensure assigned annual and semi-annual progress reports are completed, filed, and submitted to the appropriate funding source in a timely manner.
- Ensure that ISP objectives and goals are conveyed to staff and that goals are reflected in the services being offered. Ensure all staff comply with ISP objectives and goals including tracking appropriate data related to goals and communicate other tracking needs to the appropriate leadership staff.
- Ensure that activities and trainings comply with ISPs, program designs, and established curriculum. Ensure that activities are meaningful to the people served. Work with staff/consultants to adjust teaching strategies and training curriculum as necessary to support the program participants to achieve documented goals and objectives.
Additional Duties (as needed)
In the absence of a day program leadership team member or during times of transition the DPSC may also be responsible for the following:
- Oversee daily operation of Day Program services, ensuring that the people served by the program are adequately supervised, activity schedules & lesson plans are being written and carried out, and assisting with direct support to the individuals served (feeding, toileting, etc.) as needed.
- Assume delegated supervision responsibility for the Day Programs in the absence of the program Director. Act as secondary contact person for Community Care Licensing, Regional Center, and other official site visitors.
- Ensure all required participant data is documented accurately with the exception of goal progress notes. This includes daily shift notes, health checks, special incident reporting, and other needed tracking, unrelated to goals.
- Meet the staffing needs of day program services by screening for hire and providing initial orientation and ongoing training to staff.
- Evaluate and monitor the competencies and job performance of staff to include disciplinary action, when required.
- Ensure staff are in compliance with Las Trampas policies and procedures and provide disciplinary action, when required.
- Help conduct regular staff meetings and coordinate in-service training for staff. Attend other such meetings and training as may be required.
- Actively seek, develop, and integrate new and innovative activities, methodologies, technologies, materials, and other program enhancements to increase employee satisfaction and ultimately the services we provide.
- Operate the programs with fiscal responsibility including ensuring the program has adequate supplies, equipment, and training materials to conduct the program; ensure staffing ratios are met, review excessive staff time off, maintaining and sharing accurate attendance records with appropriate parties, etc.
- Help staff coordinate logistical needs regarding pre-vocational project needs, supply purchases, planning community outings, and other needs required to implement lesson plans and meet participant goal objectives.
- Monitor and ensure the safe, effective, and responsible use of agency vehicles for transportation of individuals served by the programs. Report maintenance concerns, damage, and other issues.
- Monitor and address facility needs with assistance from the Maintenance department and appropriate cleaning crews. Act as the community liaison to help foster a positive relationship with our neighbors including addressing any complaints from the community and helping organize community hosted events at our facility.
In addition to the duties noted above the DPSP shall also be responsible for all other duties assigned by their supervisor.
Qualifications

Associate's degree or higher desired (experience may be substituted for education)

A minimum of two years of successful supervisory experience in a social work or other related field is desired

A minimum of two years of experience working with people with developmental disabilities

Experience in behavior management preferred with a demonstrated understanding of positive behavior support principles
