Care Coordinator - Haitian Creole
Lasante Health Center is seeking a Care Coordinator to join our team. The ideal candidate will monitor our patients’ medical treatment and facilitate communication with healthcare providers to guarantee access to high-quality medical care.
Responsibilities:
- Regularly monitor and manage a designated group of patients with chronic medical conditions.
- Enroll patients in different medical programs
- Engage in consultations with patients and their family members to address health concerns.
- Provide comprehensive education to patients regarding their medical conditions, medications, and specific care protocols.
- Assist patients in scheduling appointments
- Complete other tasks as directed by supervisors.
Qualifications:
- Customer service experience
- Computer skills
- Ability to handle multiple tasks concurrently
- Care coordination experience preferred
