LaSante Health Center is seeking Telehealth NP / PA to deliver essential healthcare services to patients through telemedicine platforms. This role involves collaborating with healthcare teams, assessing patient requirements, diagnosing conditions, and devising treatment strategies.
Responsibilities :
Plans and organizes patient care according to protocols and pathways.
Collaborates with the healthcare team to develop and implement treatment plans.
Conducts health assessments and orders necessary tests.
Prescribes medications and medical equipment as per practice protocols.
Evaluates treatment effectiveness and modifies plans as needed.
Promotes professional growth and serves as a clinical resource.
Assists Clinical Access Center staff when necessary.
Completes additional tasks as directed by supervisor
Qualifications :
License : Current registration as Nurse Practitioner in New York State
Education : Master's degree in nursing
Certification as an Adult or Family Nurse Practitioner
