Referral and Medical Records Coordinator
Lasante Health Center is seeking a Referral and Medical Records Coordinator to join our team. The ideal candidate will ensure continuity of care by managing referrals, prior authorizations, and medical records.
Responsibilities :
- Manage outgoing referrals based on patient needs and provider recommendations.
- Follow up with patients to confirm appointment scheduling and attendance.
- Communicate with external providers to obtain necessary documentation.
- Process prior authorizations for medications, specialist visits, and procedures.
- Track and resolve issues related to prior authorization requests.
- Upload medical records into our EHR accurately and promptly.
- Respond to medical records requests from patients and authorized entities.
- Assist patients with inquiries regarding referrals and authorizations.
- Maintain accurate logs of referral activities and medical records requests.
- Collaborate with clinical and administrative staff to ensure effective communication.
