The Registered Nurse (RN) is essential in our mission to provide patients with a top-tier dialysis treatment experience. As an integral team member, the RN will work closely with CHTs, Doctors, Management, patients, and families to improve their quality of life. Qualification

· Ability to operate hemodialysis machines and equipment.

· Strong communication skills to effectively interact with patients, families, and healthcare professionals.
· Ability to work in a fast-paced environment and handle multiple tasks simultaneously.
· CA RN license is required (Willing to train new graduates).
Responsibilities:
· Responsible for ensuring a welcoming, friendly, and helpful work environment rooted in a high standard of patient care.
· Team members must participate in the company's target goals for patient clinical outcomes, quality care, a positive team environment, and customer service.
· Perform the nurse's job responsibilities per facility protocol and state and federal regulations.
· Attend and participate in staff meetings/in-services to ensure continuing education to comply with company policy and procedure.
· Collaborate with interdisciplinary healthcare teams to coordinate patient care.
· Educate patients and their families on End-stage renal disease and treatment compliance.
· Ensures the hemodialysis treatments are in accordance with approved facility policies and procedures.
· Ensures the Doctor's orders are followed, such as the correct setup of the dialysis machine and the use of proper supplies.
· Perform and Review patients' routine laboratory work Labs, discuss with Nurse Manager & MD, and implement a plan of action.
· Will collaborate with an interdisciplinary team to review Patient treatment data, review trends, and perform root cause analysis. The Candidate will participate in the monthly QAPI Meeting.
· Perform comprehensive assessments of patient's health status and develop individualized care plans.
· Maintains CIPAs and POCs on all new admissions and ongoing for established patients. Participate in Patient care conferences, medical rounds, and chart reviews.
· Administer and document all prescribed pre/ intra and post dialysis medications and charge items in the EMR system.
· Documents patient education to ensure compliance with the local ESRD Network, CMS, and clinic requirements.
· Maintains Patients' electronic medical records (patient chart) to ensure continuity of care, including monthly progress notes.
· Monitor and record patients' vital signs and medical history.
· Ensure strict reinforcement of infection control practices.
· Manage the day-to-day flow of the clinic.
· Oversee the facility's clinical procedures.
· Ensure Staff is treating Patients with respect and care.
· Adhere to CMS and DHS regulations along with Company Policies and procedures.
· Willing to train!!!
