- Collect and document patient information, including medical history and current condition.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with other healthcare professionals to ensure seamless care delivery.
- Monitor patient progress and make recommendations for adjustments to care plans as needed.
Requirements:
- Must have a valid LPN/VN license and current BLS/CPR certification.
- Must have experience working in a healthcare setting, preferably with geriatric patients.
- Must possess excellent communication and interpersonal skills.
