About the Position
We are seeking a licensed social worker (LSW) to join our team at Ciena Healthcare. As a dedicated professional, you will provide medically-related social services that help seniors attain and / or maintain their physical, mental, and psychosocial well-being.
Key Responsibilities
- Conduct admissions and social services evaluations to identify residents' needs.
- Provide financial and legal assistance, such as applying for pensions or referrals to lawyers.
- Develop discharge plans to ensure safe transitions to community congregate living or other facilities.
- Arrange counseling services and psychiatric medication management as needed.
- Maintain accurate and timely documentation in MDS care plans.
