The Supportive Housing Services (SHS) Traditional Health Worker (THW) provides peer support services and resident advocacy at a Permanent Supportive Housing complex, The Nel. The Nel will operate under the Housing First model of tenancy. The SHS Traditional Health Worker/Peer Support Specialist will work in collaboration with care coordinators, case managers, community providers, and housing provider to advocate for persons with disabilities while they work to obtain and maintain permanent supportive housing.
Here's what the job of a Traditional Health Worker/Peer Support Specialist looks like: You'll assist participants in achieving their housing goals, completing applications for supportive and subsidized housing, and advocating for them with landlords and service providers. You'll monitor and support participants' progress in their housing plans, providing skills training in medication management, financial management, and daily living activities to maintain permanent supportive housing. Additionally, you'll complete required documentation within established timelines and conduct activities to help residents develop skills for maintaining residency. Your role will also involve working with community agencies, securing resources, collaborating with staff and family members, participating in team meetings, developing support groups, and providing daily check-ins and face-to-face support for residents. Minimum Requirements:
- May hold current registration on the Traditional Health Worker Registry as Traditional Health Worker (PSS, CHW, PWSS)
OR
- Have the opportunity to obtain Traditional Health Worker Certification within one year of hire
OR
- Lived experience of homelessness, mental health, and/or substance use plus a combination of 2 years of related employment, education, or volunteer work with a goal of Traditional Health Worker certification
OR
- A combination of lived experience, employment, and education
AND
- Valid Oregon Driver License in accordance with Laurel Hill Center vehicle use policy and procedures
- Documentation of successful completion of a background and fingerprint check as specified by Oregon law, as well as a urinalysis drug screen
Salary : $22 - $28
