Responsibilities
- Develop and implement individualized care plans for residents, ensuring their needs are met effectively.
- Coordinate with healthcare professionals and community staff to provide comprehensive support.
- Monitor resident progress and adjust care plans as necessary based on ongoing assessments.
- Facilitate communication among families, residents, and staff to ensure transparency and understanding.
- Assist in conflict resolution among residents or staff to maintain a harmonious environment.
- Provide training and support for staff on best practices in resident care and management.
- Stay informed about regulations and best practices within the healthcare industry to enhance service delivery.
- Promote a culture of compassion and respect within the community.
Qualifications
- Proven experience in case management or care coordination within healthcare settings.
- Strong knowledge of senior living principles.
- Excellent communication skills for effective collaboration with residents, families, and staff.
- Ability to develop comprehensive care plans tailored to individual resident needs.
- Experience in conflict management and resolution strategies is beneficial.
- Familiarity with first aid procedures and emergency protocols is a plus.
- Background in social work or direct support roles is preferred but not required.
