What are the responsibilities and job description for the LPN - Personal Care Center - Part-time 6am-2pm & 2pm-10pm position at Laurel View Village?
Purpose:
The Personal Care Supervisor supervises the day to day nursing act performed by the Resident Assistants, and provides direct nursing care to the Personal Care residents. The PC Supervisor ensures the optimum level of care delivered to the residents in compliance with current Federal, state, local and facility standards, guidelines, regulations, and policies and as may be directed by the Personal Care Coordinator.
Reports To: Personal Care Coordinator or Personal Care Administrator
Primary Job Duties:
Directs and renders care to the residents within the Personal Care Center. Assists the Personal Care Coordinator with admissions, transfers, and discharge of residents.
Administers medications, charts, assists physicians, and supervises Resident Assistants according to established policies and procedures.
Reviews medication orders and medication sheets for completeness and accuracy of information, accuracy in the transcription of physician orders, and adherence to stop order policies.
May determine employee work assignment and has the authority to make changes and revise duties as needed to meet resident needs.
Makes daily visits to resident’s room to observe and evaluate physical and emotional status/needs of the residents during shift. Observe resident condition and notify the PC Coordinator or M.D. of any change or any possible need for medication or treatment.
Provides direct resident care when needed, i.e. takes blood pressure, weights, assists with transferring of residents, etc.
Does charting and completes other required documentation.
Receives and understands resident reports from staff on previous shifts.
Administers prescribed medication and treatments ordered by the physician. Chart the same and note effects. Note: LPNs can only administer treatments that they are authorized to administer.
Keeps the PC Coordinator informed of resident status and other related matters via written reports and verbal communication.
Interprets and implements facility and department policies and procedure in a fair and consistent manner.
Notifies appropriate family members or responsible parties of any change in the resident’s condition, if supervisor of shift.
Completes incident reports promptly for residents and staff when necessary.
Keeps privileged information about residents confidential according to HIPAA guidelines.
Upholds Laurel View Village’s Code of Conduct and follows Corporate Compliance guidelines per the LVV Employee Handbook.
Converses with residents and their families regarding problems and possible solutions, progress and plan of care, and convey a genuine sense of concern.
Requests necessary equipment and supplies as needed.
Improves professional knowledge through professional reading, in service education and other avenues. Attends in-service education and supervisory meetings on a regular basis.
Answers telephone inquiries in a professional manner.
Assists in the orientation of new Personal Care staff.
Orders prescribed medications, supplies and equipment as established by facility policies and procedures.
Performs all duties in a safe manner and follows infection control, isolation and universal precautions, practices and procedures.
Follows facility complaint procedure and adjusts or suggests solutions to resolve problems.
Orients new residents and their family members to functions of the Department.
Forwards diet changes to Dietary Department.
Performs other duties as necessary and appropriate, or as directed by the PC Coordinator or Personal Care Administrator.
Utilizes computer equipment and keeps up to date on computer for hours EMAR/EHR
Completes annual compliance and HIPAA training and exhibits behavior as set forth by the Code of Compliance in the performance of their duties.
Adheres to the Organizations Policies and Procedures regarding the Compliance Program of the organization and is timely in reporting any possible concerns.
Follows all local, state and federal regulations as they pertain to their position.
Adheres to the Organization’s Policies and Procedures covering Privacy and Security in Compliance with HIPAA regulations.
Education and Experience Requirements:
- Must be a LPN licensed in the state of Pennsylvania.
- Must have prior experience in long term, personal or acute care.
Other Requirements:
- Must deal tactfully with staff, residents, family members, visitors, government staff, and the general public.
- Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to Personal Care.
- Must possess supervisory ability, good communication skills, and the willingness to work harmoniously with professional and non-professional staff.
- Must have the ability to plan, organize and schedule work tasks and duties for assigned staff; and ability to interpret program, goals, objectives, policies and procedures necessary to providing quality care to assigned staff.
- Must be able to project a positive job attitude.
Working Conditions:
- Sits, stands, bends, lifts and moves throughout facility during workday.
- Subject to exposure to infectious waste and disease (HBV and HIV).
- Must be able to cope with emotional and hostile residents, families, guests, staff and other visitors.
- May be required to work after regular work hours, on holidays, and on weekends.
- May be required to lift a minimum of fifty (50) pounds.
