Nutrition Specialist

Responsibilities:

• Develop and implement comprehensive nutrition plans of care for residents/patients in collaboration with the interdisciplinary team.
• Collaborate with the healthcare team to provide diet instruction, education, and support to residents/patients, family members, and caregivers.
• Ensure timely and accurate documentation of nutrition assessments, recommended interventions, and follow-up actions.
• Monitor and address nutrition-related concerns with the interdisciplinary team.

Requirements:

• Previous experience as a registered dietitian or consulting dietitian preferred.
• Possess excellent communication and interpersonal skills to work effectively with patients, families, and healthcare professionals.
