Transition Coordinator for Special Services
SUMMARY: Responsible for planning and implementing the district’s special education transition program.
ESSENTIAL DUTIES AND RESPONSIBILITIES. Other duties may be assigned.
- Audit all folders for high school students to determine that transition documents are in
- Track transcripts for high school special education students.
- Conduct vocational assessments.
- Participate in the development of students’ transition goals and objectives.
- Participate in the secondary students’ Admission, Review and Dismissal committee meetings.
- Participate in the secondary students’ Individual Transition Plan meetings.
- Coordinate with agencies concerning their attendance at students’ IEP meetings.
- Coordinate with the Career and Technology Education staff in order to facilitate secondary special education students’ participation in CTE classes.
- Identify/develop job opportunities, assist students with completion of applications, participate in job interviews, and provide job coaching according to the individual needs of the students.
- Train students in appropriate work-related attitudes, habits, behaviors, dress, and communication skills necessary to obtain and maintain employment.
- Evaluate student vocational skills and behavior to assist in determining work readiness, abilities, interests, and preferences.
- Disseminate information to parents and students on the purpose and expected outcomes of participation in transition planning.
- Disseminate information to parents and students on the purpose and expected outcomes of work-based learning.
- Provide a continuum of supports and services to employers of transition
- regularly scheduled job site visits;
- on-going communication (written correspondence, phone calls, e-mail, etc.)
- on-the-job supports/job coaching
- Collaborate with employers to provide learning opportunities for transition.
- Maintain and document ongoing contact with employers.
- Network with local business and community leaders to gain knowledge of the job market in the community and help develop job sites.
- Collaborate with other service providers in building employability skills for transition students.
- Maintain an active role in developing positive public relations between school and community.
- Conducts regular meetings with professional staff and other interested parties.
- Coordinates departmental operations with other school district departments.
- Participates in special consultations and case conferences with psychologists, other student services personnel, building administrators, teachers, parents, and as appropriate with students.
- Provides opportunities for staff development
- Perform all duties in a safe manner to avoid injury to oneself and/or to others.
- Performs other such tasks and assumes other such responsibilities as may from time to time be assigned.
SUPERVISORY RESPONSIBILITIES:
Coordinates the activities of the Transition Services Staff.
QUALIFICATION REQUIREMENTS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. EDUCATION and/or EXPERIENCE:
Masters Degree with intensive course work and/or experience in teaching, psychological services, testing, guidance, and/or administration. Must have valid teaching certificate, at least five years' successful experience in public school work either as a teacher or in psychological services, testing, guidance, and/or administration.
MENTAL DEMANDS:
- Requires travel to various sites throughout the district
- Requires ability to provide instruction in a variety of environment
- Requires travel within the community for student job development, supervision and evaluation
- Home visits to parents
LANGUAGE SKILLS:
Ability to read, analyze, and interpret scientific and technical journals, financial reports, and legal documents. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information to administrators, top management, public groups/community, and Board of Education.
MATHEMATICAL SKILLS:
Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.
REASONING ABILITY:
Ability to define problems, collect data, establish facts, and draw valid conclusions.
OTHER SKILLS and ABILITIES:
Must have experience in or have knowledge of the various handicapped conditions, procedures for identifying these conditions and program alternatives for handicapped children. Ability to develop effective working relationships with students, staff and the school community. Ability to communicate clearly and concisely, both orally and in writing. Ability to perform duties with awareness of all district requirements and the Board of Education policies.
TERMS OF EMPLOYMENT
- One hundred ninety (190) days per year (4 days per week)
- Salary is dependent upon placement on the Laurens School District No. 55 salary scale.

- Performance of this job will be evaluated in accordance with provisions of the Board’s policy on Evaluation of Professional Staff.
