LaVigne Home Mission Statement: Provide exceptional end of life care and support to terminally ill people in their final days of life, regardless of income and/or ability to pay.
POSITION: Caregiver– FULL TIME - Salaried
REPORTS TO: Executive Director
SPECIAL CONDITIONS: May requires some flexible scheduling to meet the needs of guest caregiving – including some evening, weekend, and on-call.
The Compassionate Caregivers of the Lavigne Home will work under the direction of the Executive Director. Caregivers are responsible for ensuring guests receive exceptional care according to the plan agreed upon between the guest’s family and the hospice agency responsible for medical and nursing care. The caregiver also works closely with the Director to ensure appropriate staff coverage and training needs are met.
RESPONSIBILITIES OF THE COMPASSIONATE CAREGIVERS
· Plans and helps coordinate guest care among hospice agency, caregivers, and guest’s family.
· Assures adequate preparation for, and receives new guests and their families.
· Provides and documents an initial assessment of guest’s symptoms and coordinates with hospice provider to maintain accurate records of care.
· Helps facilitate and assist with training with skills workshops for all staff, including on the job.
· Contacts local fire rescue for lift and assist if a guest falls and cannot get up without assistance.
· Follows all medication policies as directed
· Dispenses and documents appropriate medication following hospice orders.
· Maintains accurate medication counts on appropriate medications
· Oversees dispensing and recordkeeping of comfort medications by volunteers/caregivers
· Prepares guest meals as appropriate.
· Communicates any issues related to guests and/or their families to the Director.
· Light housekeeping duties.
· Some care duties may include personal care of guests which may include emptying catheters, changing briefs, repositioning guests, and emptying colostomy bags.
· Lifting, repositioning, and assisting with ambulation of guests per plan of care.
· Other duties as directed by Director and Board of Directors
QUALIFICATIONS
· Previous experience with care giving.
· Compassionate heart for terminally ill guests and their families.
· Solid understanding of hospice care and basic homecare skills.
· Ability to respond to needs of guests and families requiring attentiveness to physical, emotional, and spiritual needs.
· Teamwork and supportive attitude and presence.
· Written and verbal communication skills; basic computer skills.
· Knowledge and compliance with HIPPA requirements.
· Commitment to continuous learning with an emphasis on end-of-life symptom management; pain management; spirituality of end-of-life care, and non-profit management
