Position Summary
The Senior Day Services Nurse is a registered nurse or licensed practical nurse who is the lead clinical staff person in the Senior Day Services program. She works with the Program Director to ensure that SDS participants are regularly assessed and appropriately attended to for their clinical needs. This includes provision of health-related services appropriate to the client's needs as identified in a provider's assessment and orders, including health monitoring, nursing intervention on a moderate or intermittent basis for medical conditions and functional limitations, medication monitoring, medication administration or supervision of self-administration, and coordination of health services. She also serves as a teacher, advocate, and navigator for participants and their family members/caregivers and assists in training support staff. This position reports to the Director of Nursing.
Qualifications
· Strong patient assessment skills required including ability to triage patients. Ability to use the nursing process to guide patient encounters and care.
· Interpersonal skills, including an ability to work well with the variety of ages, cultures, and temperaments represented among SDS staff and participants, treating others with kindness and professionalism.
· An ability to communicate effectively with participants in a manner that protects their confidentiality and dignity, and is sensitive to their culture and physical, emotional, and spiritual condition.
· A commitment to demonstrating personal integrity through punctuality, honesty, an ability to follow instructions, proper attention to detail in all work matters, and a willingness learn from others.
· An ability to work independently, take initiative, and set priorities in accordance with the needs and mission of the SDS program.
· Physical stamina and an ability to be standing, active, and in motion most of the day. Strength and dexterity to move patients, handle equipment, and move quickly to fulfill job responsibilities.
· Conscientious of departmental and organizational policies and procedures, and able to embrace and personify the mission of the Lawndale Christian Health Center.
Required Knowledge, Education, Experience or Licensure/Registration
• RN or LPN Licensure in the state of Illinois
· Certified in CPR and trained in first aid
· Experience with a geriatric population preferred
• Knowledge of Spanish preferred but not essential
