We are seeking a dedicated and compassionate Physical Therapist Assistant to join our LCMH team. In this role, you will work under the supervision of licensed physical therapists to assist in the rehabilitation of patients recovering from injuries, surgeries, or chronic conditions. You will play a vital role in implementing treatment plans, providing patient care, and ensuring that patients achieve their rehabilitation goals in various settings including outpatient clinics, acute care facilities, and home health environments.
Responsibilities
- Assist physical therapists in developing and implementing individualized care plans for patients.
- Provide direct patient care, including administering therapeutic exercises and modalities as prescribed such as exercise, heat, cold, massage and mobilization.
- Monitor patient progress and report changes to the supervising physical therapist.
- Educate patients and their families about treatment plans, exercises, and home care strategies.
- Maintain accurate documentation of patient treatments and progress notes in EHR.
- Participate in discharge planning to ensure continuity of care after therapy sessions.
- Collaborate with other healthcare professionals to optimize patient outcomes.
- Ensure a safe and clean environment for both patients and staff.
Qualifications
- Completion of an accredited Physical Therapist Assistant program. IL PTA license required.
- Knowledge of anatomy, physiology, and principles of physical therapy.
- Experience in acute care, outpatient settings, or home health is preferred but not required.
- Strong communication skills to interact effectively with patients, families, and team members.
- Commitment to providing high-quality patient care with empathy and professionalism.
- Ability to lift 50lbs and standing for long periods of time.
