Organization:
For over 40 years, Life Enrichment Advancing People (LEAP) has been supporting people with developmental, cognitive, and intellectual disabilities to be actively involved in their communities. To this end we provide small group and individualized residential services, adult case management and training services. LEAP is a private nonprofit organization with a mission to empower people with disabilities to lead joyful lives with dignity, independence, and autonomy by providing comprehensive and individualized support. Role: Director of Case Management
The Director of Case Management holds a place on the Senior Leadership team and has overall strategic and operational responsibility for supervising a team of case managers, managing their own caseload, ensuring the delivery of high-quality services, and managing all aspects of the case management process for adults with intellectual disabilities and autism. This role requires expertise in MaineCare regulations, Targeted Case Management (TCM), Organizational Representative Payee Services, and a strong commitment to the mission of providing person-centered support to the community.
Success Factors for a Director of Case Management
The key qualifications to be successful in this position will be to exhibit exceptional leadership and team management skills. We ask that an individual in this role have a strong knowledge of community resources, service delivery models and case management best practices. The ability to handle complex and sensitive situations with professionalism, empathy and confidentiality is a requirement. We ask that candidates for this role have a valid motor vehicle operator’s license in the state of Maine with a clean driving record and be proficient in Microsoft Office Suite and case management software such as Evergreen.
Requirements
Leadership & Supervision:
o Oversee the daily operations of the Case Management department.
o Supervise, mentor, and support a team of case managers, ensuring compliance with policies, regulations, and best practices.
o Provide training, performance evaluations, and professional development opportunities for the team.
o Act as the point of contact for staff on complex case management issues, offering guidance and resolving challenges.
Program Management:
o Ensure that all Targeted Case Management services meet MaineCare Section 13 requirements.
o Ensure that all Organizational Representative Payee services meet Social Security Administration expectations and standards.
o Develop and implement case management processes, procedures, and standards to ensure high-quality service delivery.
o Monitor caseloads and ensure appropriate distribution and management of cases.
o Conduct regular audits to ensure documentation and service delivery meet compliance standards.
o Assess client needs and coordinate services such as housing, employment, health care, transportation, and social services.
Client Advocacy & Support:
o Ensure the provision of person-centered, individualized case management for clients, tailored to their unique needs and goals.
o Adress immediate health and safety needs of individuals support, including Adult Protective Services and related regulation.
o Collaborate with external agencies, service providers, and community partners to advocate for clients.
o Lead and promote the integration of clients into their communities, fostering independence and empowerment.
Compliance & Reporting:
o Ensure all case management practices comply with MaineCare regulations.
o Review and maintain accurate, up-to-date client records in compliance with legal, regulatory, and funding requirements.
o Prepare and submit reports to funders, regulatory bodies, and senior management.
o Lead quality assurance initiatives and continuous improvement efforts to enhance program effectiveness.
Collaboration & Communication:
o Serve as a liaison between clients, families, stakeholders, and service providers.
o Foster a collaborative and supportive work environment within the organization and with community partners.
o Participate in community outreach activities to promote services and build relationships.
Position Profile:
- Full-time, salary/exempt role.
- 3-7 Direct Reports
- Typical Monday-Friday schedule with infrequent early/later meetings to accommodate flexibility needed for some individuals supported.
- Travel required within the assigned regional area.
Minimum Qualifications:
Education & Experience (Per State Regulations these are Mandatory):
• A minimum of four (4) years’ experience working in the intellectual disabilities or autism field.
• Verifiable experience supervising staff providing services to people with intellectual disabilities and/or autism.
• Demonstratable knowledge of the public education system in Maine and training in flexible funding.
• Licensed physician, licensed physician assistant, licensed psychologist, licensed clinical social worker, licensed clinical professional counselor, licensed marriage and family therapists, advanced practice nurse, psychiatric nurse, registered nurse or a licensed social worker (master’s degree in social work) - OR a person who was employed on 8/1/2009 as a case management supervisor providing supervision under the former subsections of Section 13.

Physical Requirements for the Director of Case Management:
- Typical Work Conditions. Work is performed in a mix of office, community and residential environments (e.g., service provider locations, single family homes, apartments, etc.), driving between case locations and office-based administrative locations. Required to stand, walk, sit; talk to hear, both in person and on telephone; use hands and fingers to handle and/or feel objects or controls; ability for visual and gestural communications; reach with hands and arms. Regularly required to stoop, kneel, bend, crouch, twist torso and climb stairs.
- Physical Manipulation of Objects: Exerting up to 10 pounds of force frequently and/or 40 pounds of force occasionally, to lift, carry, push, pull or otherwise move objects.
- Other: Frequently; Work Alone and Drive a Vehicle. Occasionally: Physically escort adult individuals.

Salary : $65,520 - $86,967
