Responsibilities
Your responsibilities will include:
- Providing comprehensive assessments of patients' physical, emotional, and social needs.
- Developing and implementing individualized care plans that address patients' unique needs and goals.
- Coordinating efforts with interdisciplinary teams to ensure seamless transitions between departments and levels of care.
- Communicating effectively with patients, families, and caregivers, providing education and support to promote informed decision-making and empowerment.
Qualifications
To be successful in this role, you will need:
- A Registered Nurse (RN) license in good standing, with a Bachelor's degree in Nursing preferred.
- At least three years of experience in medical-surgical or critical care nursing, with case management experience a plus.
- Strong communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
- Excellent analytical and problem-solving skills, with the ability to think critically and make sound judgments.
