We are currently seeking Case Managers (bilingual preferred) for our office in Port St. Lucie!
The candidate will provide mental health services in the field/community to children, adolescents, families and adults throughout St. Lucie County.
Job Summary
The Case Manager plays a crucial role in providing support and guidance to clients facing various challenges. This position involves assessing client needs, developing individualized care plans, and coordinating services to help clients achieve their goals. The Case Manager will work closely with individuals from diverse backgrounds, ensuring they have access to necessary resources and support systems.
Duties
- Conduct comprehensive assessments to identify client needs and strengths.
- Develop, implement, and monitor individualized service plans tailored to each client's situation.
- Provide crisis management and intervention as needed, ensuring the safety and well-being of clients.
- Facilitate access to community resources, including Medicaid / Medicare, Section 8 housing, and behavioral health services.
- Utilize motivational interviewing techniques to empower clients in their decision-making processes.
- Mentor clients in developing skills for workforce development and personal growth.
- Supervise case management interns or junior staff as required.
- Maintain accurate documentation of client interactions and progress in accordance with organizational policies.
Skills
