We are seeking a Case Manager to work in Tifton, Georgia. The Case Manager under general supervision performs a wide range of tasks in the community to assist consumers in regaining control over their own lives while promoting the recovery process.  Utilizing psychiatric rehabilitation interventions provides skills training, education and case management to consumers and their families.  Monitors consumer progress towards treatment goals.  Extensive documentation is required; accurately charts services and consumer progress.  Must be able to work well with community agencies and function as part of a team.    Benefits:   Employee Assistance Program- We offer EAP services to all full-time and part-time employees. May have extended periods of sitting, walking, and standing.
