Job Description
This position will work within the assigned Jail-In Reach Program under the supervision of Jail-In Reach Coordinator. The Forensic Peer will work towards linkage and into services and follow up once the individual has been released to the community. The position will be utilizing assigned screening tools to identify individuals who will quality to this program including completing Need for Supportive Housing Surveys, for those individuals who need housing.
Other duties include – Providing mentoring, peer support and linkage as well as care coordination to adults with behavioral health needs including but not limited to substance use disorders, mental illness, trauma histories, and extensive involvement in the criminal justice system. The FPM will utilize lived experience to engage individuals prior to release, will participate in jail transition/release planning and will provide follow up to link individuals to treatment, medical care, and other community services and supports that are responsive to the needs of the individual. Other duties as assigned.

The position requires prolonged periods of sitting at a desk and working on a computer, intermittent physical activity, including standing, walking, bending, kneeling, and stooping. Must have the ability to lift, carry, push, or pull objects up to 20 pounds.

- High school diploma or GED.
- Associate degree in the field of human services preferred.
- Must have personal lived experience with the criminal justice system.
- Successful completion of CPS or CARES training and exam for certification as Peer Specialist or Certified Addiction Recovery Empowerment Specialist by the Georgia Department of Human Resources.
- Must have the credentials of Forensic Peer Mentor to be eligible and actively working towards it.
- Must have a valid driver’s license.
- Must be able to provide 2 letters of recommendation from someone who has had an impact on your recovery (mental health specialist, natural support, law enforcement official, etc.)
- Proficient in verbal and written communication.
- Excellent time management.
- Experience providing services to the LBHS’ target population including familiarity with relevant culture(s) and language(s).
- Relationship driven and practices equity and inclusion with all interactions with person served, co-workers, and community partners.
- Promotes and demonstrates teamwork.
- Ability to problem-solve independently.
- Uses good critical thinking skills to prioritize.
- Excellent time management and organizational skills.
- Excellent communication skills to work collaboratively in a diverse environment to provide verbal and written reports.
