Legacy Community Health is hiring immediately for Outreach and Enrollment Specialists through our partnership with the AmeriCorps Program. AmeriCorps is a Federal Program that connects Americans with Community Services in Health, Public Safety, etc.

Must commit to completing the 1700-hour requirement by August 31, 2025. There is a bi-weekly (every two weeks) stipend.

About the Position
Represents Legacy Community Health at community events, promoting accessible healthcare services. Helps form enduring partnerships with local organizations to strengthen community ties and improve healthcare accessibility. Assists patients with follow-up services to sustain these relationships and ensure continued healthcare support.

Key Responsibilities

  • Provide education to community members on available healthcare programs and services at Legacy Community Health.
  • Form relationships with local community organizations to bridge gaps in care for Legacy patients and foster stronger connections between Legacy and the communities served.
  • Assist patients in addressing financial barriers to receiving primary and preventative healthcare services through community outreach and enrollment into health insurance affordability programs such as Medicaid and the Children’s Health Insurance Program (CHIP).
  • Connect patients to primary and preventative appointment scheduling services.
  • Host outreach events to promote healthcare access in underserved community areas.
  • Provide follow-up services to patients to ensure that their healthcare needs are being met.
  • Perform other duties as assigned.
In addition to the activities above, all Legacy Community Health Corps Members will:
  • Represent Legacy’s AmeriCorps Program by wearing the AmeriCorps logo daily
  • In the event of a local, state, or federal disaster declaration, AmeriCorps members may be asked on a volunteer basis or be required by OneStar to activate to assist in disaster response and recovery activities within their communities. This may involve virtual service opportunities or in-person service opportunities. This may include service opportunities outside of the member’s standard service hours or in place of their standard service duties. In the event of a large-scale, high needs local, state, or federal disaster, AmeriCorps members may voluntarily or be required by OneStar to deploy to an area outside of their service area to assist in disaster response and recovery activities.
Nonessential Functions: Legacy Community Health Corps Members may have opportunities to engage in the following activities:
  • Work with community partners to identify service opportunities in alignment with the program design.
  • Members may be asked to perform other position-related instructions within the scope of their normal service activities as requested by their site supervisor and/or AmeriCorps program staff in alignment with the grant and not unallowable or prohibited activities and subject to reasonable accommodation.
  • Provide translation and interpretation services as appropriate.

Our goal is to treat the entire patient while improving their overall wellness and quality of life, in addition to providing free pregnancy tests, HIV/AIDS screening. At Legacy, we empower patients to lead better lives by promoting healthy behaviors and offering resources such as literacy classes, family planning services, and nutrition and weight management information. 

Our roots began in 1981 as the Montrose Clinic, with specialization in HIV education, testing, and treatment. Since then, the agency has expanded to >50 clinics in Houston, one in Baytown, two in Beaumont, and one in Deer Park with extensive services that include: Adult primary care, HIV/AIDS care, pediatrics, OB/GYN and maternity, dental, vision and behavioral health. We also service students within KIPP and YES Prep schools. Legacy is committed to driving healthy change in our communities.
Requirements

  • High School diploma or GED is required.
  • No previous experience is required.
  • Bilingual / Spanish fluency is preferred.
  • U.S. Citizen or a Permanent Resident
