Be a part of driving healthy change in our communities About the Position The Care Team Assistant at Legacy Community Health provides both clinical and clerical support to the team. You will ensure that we provide quality and compassionate evidence-based care in a Patient Centered Medical Home environment by assisting with phone calls, lab results, pre-visit planning, medication management, and more. About Legacy Community Health Legacy Community Health is a premium, Federally Qualified Health Center (FQHC) that provides comprehensive care to community members regardless of their ability to pay.
