Job Description:
- Develop and implement individualized treatment plans for patients
- Assess patient health status and provide recommendations for care
- Collaborate with the Service Line Director and Clinic Site Director to improve patient outcomes
- Stay up-to-date with industry developments and best practices through ongoing education and training
- Provide exceptional patient care and service

Requirements:
- Current, unrestricted Texas advanced practice license
- Active DEA Registration
- BLS Certification
- Proficient with EMRs (Epic highly preferred)
- Minimum 2 years of experience as an Advanced Practice Provider required
- Bilingual in Spanish preferred
Estimated Salary: $120,000 - $180,000 per year
