Job Title: Advanced Practice Provider
We are seeking an experienced Advanced Practice Provider to join our team at Legacy Community Health, a Federally Qualified Health Center (FQHC) in Southeast Texas.
The ideal candidate will have 2 years of experience as a Nurse Practitioner or Physician Assistant and be comfortable working in an autonomous environment. They will be enthusiastic and passionate about Community Health and providing high-quality patient care.
**Key Responsibilities:**
- Initiates an appropriate assessment of patient's health status
- Assists the Service Line Director and Clinic Site Director in ongoing assessment of patient services
- Consults with colleagues and supervising Physicians on patient care
- Documents patient care in a timely manner
- Provides medical services to clinic patients

**Requirements:
- Current, unrestricted Texas advanced practice license
- Active DEA
- BLS
- Proficient with EMRs (Epic highly preferred)
- 2 years of experience as an Advanced Practice Provider required
- Bilingual in Spanish preferred
**Estimated Salary:** $120,000 - $180,000 per year
