Job Duties

This position involves providing primary care services to patients, conducting assessments, developing treatment plans, and ensuring timely provision of necessary healthcare services.

Main Job Functions:

• Evaluate patient health information, collect additional data, and modify treatment plans as necessary.
• Document patient care in a timely manner, maintaining legible and concise records.
• Promote health education and awareness among patients and communities.
• Maintain accurate and up-to-date patient records.

Required Education and Certifications:

• Current, unrestricted Texas advanced practice license.
• Active DEA registration.
• BLS certification.
• Proficiency with EMRs (Epic highly preferred).
