Summary
As the Social Service Designee you will assist the Social Service Director in the planning, developing, organizing, implementing and directing of the social service programs of this facility to assure that the emotional and social needs of the residents are met.
You will assist in reviewing complaints made by the residents, assist residents with information concerning their rights, living wills, etc. The Social Services Designee will make routine visits to residents and maintain contact with resident’s family, involving them with non-medical progress reports. You will assist in providing solutions for social and practical environmental problems including seeking financial assistance, discharge planning (including collaboration with community agencies), and referrals to other community agencies when specialized assistance is required.
Responsibilities:
Completes comprehensive psychosocial interviews and assessments of residents and families.
Develops comprehensive care plans to address any psychosocial needs of the residents.Coordinate and assist with scheduling and facilitating care conferences.
Help residents/families adjust to residents' current level of care and provide support as needed.
Educates residents/families on the levels of health care or any healthcare changes, community resources, discharge planning, and discuss advance directives to obtain wishes regarding those decisions.
Promotes communication and collaboration among health care team members
Daily documentation of interactions and treatment in accordance with facility, state and federal compliance rules and policies.
Documentation of discharge planning efforts and coordination of care with outside agencies.
Complete MDS and follow RAI guidelines.
Maintain communication and report on changes in health, current goals, discharge planning, and encouragement to participate in care planning.
Network with social, health and community agencies and be responsible for referrals of residents when appropriate.
Assist the resident by ordering durable medical equipment as needed.
