Responsibilities
The Pharmacy Transitions of Care Technician, under the direct supervision of a pharmacist, will assist the care team through various phases of the patient’s care including but not limited to: 1) Gathering the best possible medication history that is then utilized by the care team for medication reconciliation, and 2) Facilitates the provision of medications at the time of discharge from the hospital.
Qualifications
Education:
- High school diploma or equivalent required.
- Graduate of a pharmacy technician program preferred.
Experience:
- Minimum 2 years of experience as a pharmacy technician required; retail and/or inpatient hospital experience preferred.
- Experience applying customer service behaviors and communication skills required.
- Basic familiarity with computers and keyboard skills and ability to navigate electronic systems applicable to job functions.

Licensure
Licensure/Certification:
- Current applicable state Pharmacy Technician Licensure OR Pharmacy Intern License required prior to date of hire and maintained thereafter.
Pay Range
USD $24.83 - USD $35.50 /Hr.Salary : $25 - $36
