Were looking for a Community Outreach Specialist who is passionate about community outreach for our PAS program.
Your Role :
- Identify potential patients / clients who can benefit from our services.
- Engage with patients / clients, their families, caregivers, and community members to ensure they are satisfied with our care.
- Develop and implement programs to enhance our community outreach efforts.
- Participate in community events, health fairs, and clinics to raise awareness.
- Build strong relationships with local doctors offices, community centers, senior living facilities, and programs.
