HOSPICE ON-CALL TRIAGE NURSE & RUNNER - Monday to Thursday 5pm to 8:30am

Legacy Hospice and Palliative Care, located in Chino, CA, is currently looking for compassionate and caring TRIAGE NURSE/RUNNER to come be a part of our team!!!

MUST HAVE WORKING EXPIRENCE AS A NURSE IN HOSPICE

We are a hospice and palliative care agency seeking a per-diem TRIAGE NURSE/RUNNER who will have a crucial role in assessing patient needs, prioritizing care based on urgency, and ensuring that patients receive the appropriate medical attention in a timely manner. This position requires strong clinical judgment, excellent communication skills, and the ability to work effectively in a fast-paced environment.

  • Must have working knowledge of HospiceMD.
  • Must have hospice care experience.
  • As an on-call person you will receive a standby rate in addition to your visit rate.
  • Monday to Thursday 5:00pm to 8:30am cover all areas
  • Per-diem, the pay rate is determined as per visit.

BASIC REQUIREMENTS

1. Respond to patients, families, caregivers, and community partners, and interact with Supervisor and co-workers in a calm, caring, supportive and respectful manner at all times.

2. Communicate clearly and concisely through verbal, listening and written means.

3. Demonstrate excellent clinical assessment and critical thinking skills.

4. Receive direction and follow through with minimal supervision.

5. Organize work activities, multi-task, and remain flexible with assignments.

6. Possess strong interpersonal, educational and collaborative skills.

7. Must participate in 1 week per month of on-call duties.

DUTIES AND RESPONSIBILITIES

1. Treat patients and their loved ones in a compassionate, dignified and kind manner at all times.

2. See death as a natural part of life and accept the hospice philosophies of: quality rather than quantity of life; comfort when there is no cure; the right of self-determination

3. Exhibit safety consciousness at all times and report unsafe conditions to the Referral and Admissions Manager and the person within the organization responsible for safety issues.

4. Promote and coordinate multidisciplinary care provided in an interdisciplinary fashion, engaging with other professional care team members as needed to provide full and effective service in the physical, emotional, social and spiritual dimensions of care.

5. Collect documentation for all new referrals, and communicate with patient/family, Hospice Administrator, and referral sources.

6. Respect, promote and protect patients’ rights to individualized care and collaboration in the Care Planning process.

7. Connect with other members of the Care Team and external medical community partners to ensure continuity of care.

8. Communicate regularly with the Referral and Admissions Manager and other care team members to review unique patient issues, share professional support, and exchange feedback aimed toward enhancing patient-centered care.

9. Understand, support, and comply with all Federal and State regulations including HIPAA, CMS Conditions of Participation (CoPs), LHP Policies and Procedures, and accepted standards of nursing practice. Always record legible, clear, complete and dated documentation.

10. Learn the EMR and other technology required to fulfill the duties of the position.

11. Ensure that the clinical records, with all the information contained therein, are safeguarded from loss or unauthorized use.

12. Complete LHP On-boarding and Orientation and the Initial Competency Skills checklist with Immediate supervisor within 60 days, and receive the support of a preceptor as needed.

13. Demonstrate continued professional growth and development through participation in staff training, internal and external educational programs and review of current health care literature.

14. Share in providing 24-hour, seven-day-a-week coverage to patients/families, including afterhours and weekend support as needed, and assigned.

15. Attend required meetings such as Nursing, training, All Staff, etc.

16. Report for duty at scheduled time, ready to perform work functions, and remain on task until assignments are completed. Adhere to agency requirements regarding absenteeism and tardiness.

17. If an employee decides to terminate employment with LHP, a personal letter of resignation is required, and the agency makes the request that as much advance notice time as possible can be given, preferably at least 30-days, to ensure a smooth transition and continuity of care for the patients and loved ones.

  • Performs visits as needed and directed by the triage team. Responds to requests for patient visits in a timely manner, Independently assesses patient/family immediate care/symptom management needs. Provides care and treatment as ordered. Provides health care instruction to patients, caregivers and family members. Other Hospice nursing visits as assigned.
  • Provides appropriate follow-up to case managers and team members in regards to patient condition changes and needs.
  • Responsible for the direction of care provided by LVN and HA in accordance with the patient's plan of care.
  • Responsible for the supervision of services provided by the LVN and HA per TEH policy and regulatory requirements.
  • Maintains timely patient documentation using “point of care” electronic charting.
  • Keeps current with all organization and department policies and procedures.
  • Collaborates with Physicians and Supervisors for changes in orders and changes to the patient’s level of care as needed
  • Adhering to the E-way standards is a requirement in performing all job duties.

QUALIFICATIONS

1. Must possess and maintain a current license to practice as an LVN in the State of California.

2. Must possess a valid California Driver’s License, dependable transportation and proof of automobile insurance coverage.

3. Must have current CPR and Basic Life Support certification.

4. Must have the ability to work in diverse and unpredictable surroundings in personal residences, nursing homes, assisted living facilities, adult foster homes, etc.

5. Must be able to safely negotiate in a variety of environments, e.g. steep stairs, uneven surfaces, cramped physical quarters, etc.

6. Must be able to meet the physical demands of the position including lifting a maximum of 50 lbs, repositioning and transferring of patients, 2-person assist, providing personal care, etc.

7. Must be willing and able to drive in a variety of road and weather conditions, day or night.

  • Must have a minimum of 12 months experience with 1-2 years preferred in acute care; or equivalent combination of education and experience.
  • At least 12 months experience with 1-2 years preferred in acute care.
  • Hospice experience preferred
  • Current CA LVN license.
  • Current CPR certification and negative TB skin test.
  • Bi-lingual (Spanish) preferred.
  • Excellent customer service skills.
  • Current California driver’s license with proof of auto insurance required.
  • Because this is a field position, one must be able to drive in various conditions, including but not limited to winding/steep roads, night driving, and rural areas, and be able to enter homes with pets.

Pay: $45.00 - $55.00 per hour
