About the Role:
The Hospice RN Case Coordinator will play a vital role in planning, organizing, and directing hospice care in patients' homes and communities. As a member of our interdisciplinary team, you will work closely with patients, families, and healthcare professionals to provide unparalleled care during the most crucial stages of patients' lives.
Responsibilities:
- Conduct comprehensive assessments of patients and families to determine hospice care needs.
- Develop, initiate, and revise care plans based on patient needs and status.
- Administer medications and treatments as prescribed by physicians.
- Offer counseling and support to patients and families, including education on self-care techniques.
Qualifications:
- Graduate of an accredited nursing school with 1-2 years of recent acute care and home care experience, current state licensure, and Active CPR certification.
- Valid driver's license, personal automobile, and insurance coverage.
- One year of hospice care experience is preferred.
- Case Management experience and supervision of hospice aides is a plus.
Compensation:
Our estimated salary for this role is $90,000 - $100,000 per year, depending on experience and qualifications.
