Position Summary :
A Social Worker is responsible for providing medical social services to promote physical, medical, emotional, spiritual, and psychosocial well-being of the hospice patient. This is performed in accordance with physician orders and the patient's plan of care under the direction of the Medical Director and Administrator.
Position Responsibilities for a Hospice Social Worker :
- Understands and adheres to established policies and procedures.
- Participates in peer review and Quality Assessment and Performance Improvement as assigned.
- Participates in Interdisciplinary Group meetings.
- Takes on-call duty nights, weekends, and holidays, as assigned.
- Completes and submits assessments and reassessments in condition in accordance with defined time frames.
- Comply with all applicable Federal and State laws and regulations, including HIPAA.
- Supportive of individual and company performance improvement initiatives
- Maintain confidentiality with patients, employee, and company sensitive information.
- Comply with accepted professional standards and practices.
- Excellent communications skills : oral, written and presentation.
- Ability to interact and communicate with all levels of management.
- Ability to adjust the message to the audience as appropriate.
- Strong "closing" and "needs assessment" skills.
- Professionalism with consistent ability to balance urgency and decisiveness with patience and compassion.
Knowledge, Skills, and Abilities : Master of Social Work (MSW) degree from a school of social work accredited by the Council on Social Work Education; OR A baccalaureate degree in social work from an institution accredited by the Council on Social Work Education; OR A baccalaureate degree in social work, psychology, sociology, or other field related to social work and is supervised by a MSW meeting qualification above; AND At least one (1) year of social work experience in a health care setting.
