DAILY RESPONSIBILITIES
As the Memory Care Nursing Coordinator, you will oversee the daily operations of our Memory Care unit, ensuring that every aspect aligns with our high standards.
- Assessment and Care Planning:
- Conduct initial and ongoing assessments of residents' cognitive and physical abilities.
- Develop and update individualized care plans based on assessments and staff input.
- Oversee caregiving staff to ensure that all residents receive appropriate care and support.
- Provide training and guidance for staff on best practices for memory care and communication techniques.
- Engage residents in meaningful activities that promote cognitive function and social interaction.
- Facilitate one-on-one or group activities, such as memory games, arts and crafts, and physical exercises.
- Serve as a point of contact for residents' families, providing them with updates on their loved ones' progress and any changes in care plans.
- Offer support and resources to families dealing with the challenges of memory-related conditions.
- Maintain accurate records of residents' progress, incidents, and changes in condition.
- Prepare reports for management and attend meetings to discuss residents' needs and care strategies.
- Work with physicians, therapists, and other healthcare providers to integrate medical care with daily living activities.
- Ensure that all care meets the regulatory standards and best practices for memory care.
- Ensure that the memory care environment is safe, secure, and conducive to the well-being of residents.
- Plan for and make adjustments in the environment as needed to support residents' needs.
Skills and Qualifications:
- Must be a certified Medication Technician
- Strong communication and interpersonal skills.
- Knowledge of memory disorders and understanding of therapeutic interventions.
- Experience in healthcare or social services, particularly in gerontology or memory care.
- Ability to manage a team and promote a positive, supportive environment.
