Legacy House of Southern Hills is seeking a highly skilled and passionate full-time Memory Care Coordinator to join our team!

DAILY RESPONSIBILITIES

As the Memory Care Nursing Coordinator, you will oversee the daily operations of our Memory Care unit, ensuring that every aspect aligns with our high standards.

  1. Assessment and Care Planning:
  • Conduct initial and ongoing assessments of residents' cognitive and physical abilities.
  • Develop and update individualized care plans based on assessments and staff input.
Supervising Staff:
  • Oversee caregiving staff to ensure that all residents receive appropriate care and support.
  • Provide training and guidance for staff on best practices for memory care and communication techniques.
Resident Interaction:
  • Engage residents in meaningful activities that promote cognitive function and social interaction.
  • Facilitate one-on-one or group activities, such as memory games, arts and crafts, and physical exercises.
Family Communication:
  • Serve as a point of contact for residents' families, providing them with updates on their loved ones' progress and any changes in care plans.
  • Offer support and resources to families dealing with the challenges of memory-related conditions.
Documentation and Reporting:
  • Maintain accurate records of residents' progress, incidents, and changes in condition.
  • Prepare reports for management and attend meetings to discuss residents' needs and care strategies.
Collaboration with Healthcare Professionals:
  • Work with physicians, therapists, and other healthcare providers to integrate medical care with daily living activities.
  • Ensure that all care meets the regulatory standards and best practices for memory care.
Environmental Oversight:
  • Ensure that the memory care environment is safe, secure, and conducive to the well-being of residents.
  • Plan for and make adjustments in the environment as needed to support residents' needs.

Skills and Qualifications:

  • Must be a certified Medication Technician
  • Strong communication and interpersonal skills.
  • Knowledge of memory disorders and understanding of therapeutic interventions.
  • Experience in healthcare or social services, particularly in gerontology or memory care.
  • Ability to manage a team and promote a positive, supportive environment.
