Social Service Director

Job Summary

The Social Service Director (SSD) supervises the social service program. The SSD evaluates the social needs of each resident and formulates a written plan of care. The
SSD refers residents and families to appropriate community resources as necessary.

Education

High School Diploma and additional training whether it be college or trade school.

Qualifications

1. Must be able to read and write with good grammar skills.

2. Is self-motivated and able to take the initiative to complete duties as specified and directed by the Administrator

3. Must have knowledge of special needs and limitations of the elderly.

4. Demonstrates leadership and managerial ability

5. Has the ability to organize, plan and supervise resident admissions, in coordination with the Administrator.

6. Maintains effective interpersonal relationships and works well with the general public.

7. Is capable of implementing changes as determined by the Administrator.

8. Is of good emotional, mental and physical health having sound judgment and high professional standards.

9. Is courteous in working with residents, families and employees.

10. Maintains a neat, well-groomed and professional appearance at all times.

11. Has a desire to work with the geriatric clientele as well as residents of any age and their current prospective care needs.

12. Must be willing to be in and/or available to the facility weekends as well as weekdays.

13. Must demonstrate ability to follow policies, procedures, practices and direction without the direct supervision of the Administrator.

14. Must demonstrate the ability to function as a cooperative team member with all disciplines in the facility.

15. Must have a pre-employment and an annual Tuberculosis screening examination as required by the state the Social Service Director is employed.

Responsibilities

The Social Service Director works with the Administrator to assure the following responsibilities are accomplished. This list is only partial and should not be interpreted as a comprehensive inventory of all duties, tasks and responsibilities.

1. Has a working knowledge of resident’s rights and is an advocate for the resident.

2. Has a working knowledge of policies within the Social Service Department and throughout the facility, and abides by these policies.

3. Completes documentation in a timely and professional manner.

4. Communicates with Department Heads and other staff when necessary to insure admissions and readmissions requirements are met.

5. Keeps confidential any and all information regarding the residents’ personal and clinical records.

6. Participates in the community-oriented events and special marketing projects as requested by the Administrator

7. Maintains confidentiality, adhering to established policies and confidentiality in regards to residents, employees, and family members.

8. Communicates with resident Ombudsman and completes required follow-up and documentation post visits.

9. Identifies and evaluates personal, emotional and psychosocial needs of residents

10. Have the ability to learn computer systems, system applications and related office equipment.

11. Promotes effective work relationships with the administrator, medical staff, personnel, volunteers, agencies, and management staff affiliated with the facility.

12. Gathers sufficient pre-admission data to determine resident level of care and the ability of the facility to properly care for the resident.

13. Completes pre-admission and admission documents and forms per policy.

14. Collects accurate data for all payer source referrals.

15. Interacts with discharge planners in an effective and efficient manner. Ask for appropriate and adequate resident information.

16. Coordinates room assignments for residents in consultation with the nursing department and administration.

17. Assist with admissions, transfers and discharges in accordance to facility policy.

18. Completes Social History and all other documentation as per policy.

19. Assist with establishing a resident plan of care to address psychosocial and spiritual needs.

20. Prepares resident identification bands, bracelets and other appropriate identifying labels as required.

21. Acts as a liaison between transferring institutions and the facility; establish a good rapport with transferring institutions.

22. Is responsible for the discharge plan of residents.

23. Assist with coordinating and follow up of Resident Council and Family Council.

24. Assist with identifying community resources as needed

25. Attends and participates in facility meetings.

26. Participates in Quality Assessment and Improvement Program

27. Participates in Quality Assessment Performance Improvement Program

28. Reports immediately to supervisor and administrator situations involving abuse, neglect, or suspicion of a crime.

29. Adheres to the Code of Conduct, Compliance Program, HIPPA Standard, and
Computer Usage Agreement.

30. Other duties as assigned by the Administrator

Professional Requirements

1. Adheres to dress code, appearance is neat and clean.

2. Completes educational requirements per policy.

3. Maintains regulatory requirements.

4. Occasional travel/overnights required for training, professional meetings, emergencies, and evacuations.

5. Reports to work on time and as scheduled.

6. Wears identification while on duty.

7. Must work throughout all areas of the facility.

8. Completes work in designated time frames.

Requires full range of body motion including:

01. Standing/Walking: Frequently. Worker will be spending considerable time up and about in the facility doing various tasks as per job description.

02. Bending/Stooping: Frequently. Worker will be bending or stooping to work with supplies, preparation of documents, filing, working with clinical records, etc.

03. Lifting/Handling: Occasionally. Worker will be lifting and handling supplies to do inventory, restocking, ordering, etc. as well as filing and auditing of clinical records.
Range of Weight: 1-50 pounds.

04. Carrying: Occasionally. Worker will be carrying clinical records, manuals, supplies,

etc. Range of Weight: 1-50 pounds.

05. Pushing/Pulling Occasionally. Worker will be pushing/pulling on various supplies storing, moving and inventory of supplies storing, moving and inventory of supplies, as well as with clinical records filing, auditing, preparing for each month, etc. Range of Weight 1-
50 pounds.

06. Balancing: Occasionally. Worker will be balancing supplies to be stored, moved from storage to using area, etc.
When moving records from active to inactive status or during auditing periods worker will be balancing legal documents that are not secured.

07. Pivoting/Turning: Frequently. Worker will be pivoting and turning to stock supplies, handle records, etc.

08. Crouching/Stooping: Frequently. Worker will be crouching/ stooping to file records, working with supplies to restock inventory of supplies, etc.

09. Kneeling: Occasionally. Worker will be kneeling to file records, stock and inventory supplies, etc.

10. Reaching Worker will be reaching to file records, stock and inventory supplies, etc.

B. Equal to shoulder height: Occasionally

11. Handling/Manual Continuous. Worker will be using fine and

Job Location

Must work throughout all nursing areas of the facility. May require working irregular hours and may be exposed to communicable diseases and/or body fluids, medical preparations, toxic substances and occasionally ionizing radiation. Occasional travel/overnights for
