Job Title: Resident Services Specialist
Position Summary:
Are you a detail-oriented, compassionate individual who thrives in a leadership role? Join our team as a Resident Services Specialist and be an essential part of ensuring the highest quality of care and service coordination for our residents. In this role, you will manage key areas like care plans, documentation, housekeeping coordination, and regulatory compliance, while working closely with residents, families, and internal teams.
Key Responsibilities:
Resident Care & Compliance:
- Oversee the creation, implementation, and updating of resident care plans, ensuring they reflect changes in residents’ needs and conditions.
- Ensure documentation (LIC 625, LIC 602, LIC 621) is completed accurately and on time, including assessments, care plans, and physician reports.
- Conduct audits to maintain compliance with state, federal, and local regulations.
- Oversee timely and accurate documentation of all records in the ALIS system.
Housekeeping & Service Coordination:
- Coordinate housekeeping services for resident rooms, ensure preferences and frequency are documented, and maintain service levels.
- Monitor and assess housekeeping effectiveness, addressing any concerns and ensuring safety and cleanliness standards are met.
Collaboration & Communication:
- Work closely with the Resident Care Coordinator, Medication Room Coordinator, and Behavioral Support Coordinator to ensure comprehensive and personalized care plans.
- Maintain clear and transparent communication with residents, families, and POAs about any changes to care plans and services.
- Facilitate family meetings to discuss care plans and address any concerns.
Leadership & Supervision:
- Supervise and support the Resident Care Coordinator, Medication Room Coordinator, and Behavioral Support Coordinator.
- Provide ongoing training, feedback, and performance evaluations for the team.
- Ensure the team is compliant with clinical policies and regulatory standards.
Quality Improvement:
- Evaluate care services and documentation processes, identifying areas for improvement.
- Address resident and family feedback, implementing corrective actions as needed to enhance service quality and resident outcomes.
Other Duties:
- Assist with incident investigations, report drafting, and maintaining emergency files.
- Stay updated on relevant regulations and guidelines, ensuring the facility adheres to all standards.
Qualifications:
Education & Experience:
- Previous experience working with senior populations is preferred, especially as an RN or LVN.
- RN or LVN License preferred.
- CPR and BLS certification required.
- Tuberculosis (TB) certificate as required by the State.
Skills & Abilities:
- Strong organizational, time management, and leadership skills.
- Ability to communicate effectively both verbally and in writing.
- Knowledge of state regulations governing residential care and memory care units.
- Ability to maintain confidentiality and respect residents’ privacy and rights.
Other Requirements:
- Criminal background check clearance.
- Fingerprint clearance as required by the State.
- Must be at least 18 years old.
- Must be able to read, speak, and write English.
Physical Requirements:
- Ability to lift up to 40 lbs occasionally and stand for extended periods.
- Ability to perform occasional heavy lifting (up to 40 lbs).
- Exposure to blood/bodily fluids is likely; adherence to universal precautions is necessary.
