Plans for discharge and aftercare. Maintains contact with referring agencies, treatment team providers, and related people, and with the community resources used for referral. Maintains treatment records. Assesses and interprets data about the consumer’s status in order to identify each consumer’s needs and provide the appropriate care, including age-related care, needed by the consumer group. Evening hours required.Legacy Treatment Services offers outpatient mental health services for those who could benefit from improvements in mental and behavioral health.
